Jack Hayes

Hayes, Jack A. 64, of Providence, died Friday May 3, 2013 at Rhode Island Hospital after a long illness. He was the husband of the late Patricia (Davis) Hayes. Born in Providence, he was a son of the late Leroy and Doris (Solomon) Hayes.

Mr. Hayes was a graduate of Johnson and Wales University and a former agent for Prudential Insurance Company.

He is survived by a daughter, Tammy Rice, a grandson Michael Kohloff, Jr., a brother Harvey Hayes and his wife Patricia, and a niece Lisa Hayes.
